Office Policies

 

Appointments

All appointments are by scheduling only. Please arrive on time to ensure your full appointment time is available. If you arrive more than 10 minutes late, your appointment may need to be rescheduled.

Cancellations and No-Shows

We require at least 24 hours’ notice to cancel or reschedule an appointment. Missed appointments or same-day cancellations will incur a $50 no-show fee. Exceptions may be made for emergencies at the provider’s discretion.

Paperwork Policy

All required intake forms and paperwork must be completed prior to your appointment.
This ensures that your provider has the necessary information to deliver the highest quality care and allows your visit to begin on time.

If forms are not completed before your scheduled appointment, your visit may need to be rescheduled.

Medication Refills

Medication refills are processed during business hours only. Please submit refill requests at least 72 hours before you run out of medication.
We do not process refill requests on weekends or holidays.
Patients must attend follow-up appointments as scheduled to receive refills.

Controlled Substances

If prescribed, controlled medications require close monitoring. Lost or stolen prescriptions will not be replaced. Misuse, diversion, or evidence of multiple prescribers may result in discontinuation of controlled medication prescribing and potential discharge from care.

Telehealth Visits

Telehealth sessions are provided through secure, HIPAA-compliant video platforms. Patients must be located within a state where the provider is licensed at the time of the session. Please ensure a quiet, private, and safe environment during telehealth visits.

Communication

Routine communication (scheduling, billing, refills) should occur via the patient portal or office phone.
Clinical questions or medication concerns should be addressed during appointments.
Messages will be returned within 1–2 business days.

Emergencies

Balanced Behavioral Health Services does not provide emergency or crisis services.
If you are in crisis or experiencing thoughts of self-harm, please call 911 or go to your nearest emergency room.

Insurance & Payment

We accept most major insurance plans, medicaid, medicare, tricare and self-pay patients.
Payment is due at the time of service unless prior arrangements have been made. Patients are responsible for any copays, coinsurance, or non-covered services.


All copayments, coinsurance, and any outstanding balances are due prior to your appointment.

If payment is not received before your scheduled visit, your appointment will need to be rescheduled.

We appreciate your understanding and cooperation in helping us maintain efficient and uninterrupted care for all patients.

Confidentiality

All patient information is strictly confidential and protected by HIPAA regulations. Information may only be released with the patient’s written consent, except where required by law (e.g., risk of harm to self or others, child or elder abuse).
